  • Please print and complete all pages and give to the Coach for the sport the athlete is participating or you can give these forms directly to the Riverside Athletic Trainer, Sarah Bell.
  • All pages must be completed to be eligible for tryouts, practices, and games.
  • You must also print and complete the entire packet to be eligible to play/practice.
